turns out, it is a common problem with many listed solutions ::

for starters:
>Tech Dreams: CREATE DATABASE Permission Denied - SQL Express 2005 Problem and Solution
>Named Pipes Provider, error: 40 - Could not open a connection to SQL Server - SQL Protocols - Site Home - MSDN Blogs

does that help??

If you are using SQL Server Express Edition then you need to add that. Otherwise PC Name / IP along with Instance Name is just fine.
