want a software program which can track my my PC usage

Is there any software program which can track my usage on my PC ie. which can tell me when did I start my computer, when did i shut it down , how much time I was on the internet, which websites i visited and for what duration, how much time I was using Adobe Reader or MS word or any other program etc. It would be nice if the program would also give a graphical representation of the above data.
Please help.
